Summary of activities

Valued Lives Foundation (Inc) has supported people with a disability/and or mental health condition and aged people to continue to live in their homes in the community. We assist people with proactive planning and develop networks of supports to people who may not have family or friends in their lives. We have provided regular hands on support to people who have found themselves in a crisis situation such as homelessness, financial crisis and assisted people to access emergency support and linked to other social services. Valued Lives has provided a range of personalised supports to enhance the capacity of people with a disability, their families and supporters, to create and live valued lives in the community, including: • Coordination of Supports • Specialist Support Coordination • Self-directed Supports and Services • Personalised Futures Planning • Supporting the development of Peer Support Networks • Peer–led capacity building workshops • Facilitating Circles of Support • Microenterprise business mentoring" Valued Lives Foundation is a user-led organisation whereby people who use our supports and services ensure the organisation upholds the values we are committed to. The majority of Valued Lives Board members (over 75%) are either people with disability or mental health condition, alongside their families and supporters. Other members of our Board are professional people who support our mission and ensure our governance arrangements meet required standards. Valued Lives Foundation (Inc) employees live our values on the ground with over 60% of our team having both personal experience of having a family member with disability. Our team are highly experienced and have strong knowledge of the NDIS. Our support workers provide hands on assistance for vulnerable people to maintain their health, well-being and independence through our mentoring and support programs. To ensure Valued Lives delivers on the vision, mission and uphold its core values, key focus areas over the last financial year have been: 1. Build and grow sustainable, quality supports and services. 2. Refine and improve required systems, infrastructure and processes to manage and sustain growth. 3. Support people and families to connect with their peers to share experiences, skills, knowledge and information. 4. Continue our focus on building a strong identity and reputation as an innovative, values based, peer led community support organisation.
