Description of charity's activities and outcomes
-We have linked patients and families with existing palliative care services and supports by creating a 'Help Enlisting Card' to assist families to access support and identify where Shannon's Bridge volunteers may be needed to 'fill the gap';
-We have assisted people with Advance Care Plans in transition to their death, and have ourselves provided approx. 30 "Shannon's Packs" of emergency symptom control equipment and instructions across Victoria;
-We have provided accredited, free training to volunteers to provide practical help, based on Palliative Care Victoria's Volunteer Training Manual and in line with "Compassionate Communities" philosophies; and
-We have provided education to the public on 'death literacy' and the transition from life to death, having presented at community consultations in Talbot, Creswick and Ballarat and participated in 'Dying Well in Hepburn" Community Forums in Daylesford, Clunes and Creswick.
Where the charity conducted activities during the 2018 reporting period
Will the charity change or introduce any activities in the 2019 period?
Changes planned
Just before the end of the year, we were awarded approx $2.5 million in funding by the state government to continue the work of our Creswick hub and replicate that work in hubs based in other areas of regional Victoria.
