Interchange is a registered NDIS provider supporting Western Australians living with disability to achieve their goals and live fulfilling lives through individualised, local, community-based support. For almost 30 years we have been putting people at the centre of everything we do. Our belief is we all belong. We are all peas from the same pod! We start by working with you to design a personalised plan. Working together we focus on your interests, your personal goals and what you need from us. Our aim is to remove barriers and increase your choices. After all, this is all about you.
> We provided individualised services in the Perth Metropolitan and Peel Regions to over 400 customers with disabilities sufficiently severe to preclude them from working to gain an income.
>We promote community inclusion with strategies to improve access to community facilities, public transport and shopping. We also assist with personal living needs to improve independence.
>We have a Shared Management service so customers and their families can share the recruitment and daily management of their support staff and support strategies.
>We have an independent living support service providing support within a customer's home and community.
> We have a seamless transition program for school leavers and continue to provide support to achieve adult learning goals.
>We are NDIS Plan Managers helping you to get the most out of your plan and save you time by dealing with your providers, managing your budget, paying your invoices, and giving independent guidance. We work with you to maximise the benefits of your NDIS plan so you can live your best life. It’s fast, easy, and best of all, you still have the freedom of choice.
